Consent and Entry Forms

 

Overview

All climbers will need the following forms completed:

  • A Level 3-5 coaching parental consent form.

  • A mobility disclaimer for Yoga Loki.

  • An unsupervised entry form.

  • A helmet waiver.

 

L3-5 Parental Consent Form

This form gives parental consent for all activities conducted within training session time. Procedures, Risk Assessments, and conversations with coaches can be provided to inform parents about the activities in the session. Forms will be renewed every year.

If there is medical information relevant to the session please make sure to make sure the coach is aware prior to each session and inform us if you feel an activity may be impacted or have an impact on your health.

 

Mobility Disclaimer: Yoga Loci

Although the mobility portion of the team sessions does not include anything that could not be covered by the normal disclaimer, since we are bringing in an external instructor we ask that you also complete his paperwork as well.

 

Unsupervised Entry Form

Levels 3-5 of Team Foundry are when we really expect a degree of autonomy and personal responsibility from the climbers. To accompany the increase in session length to 2.5hrs we also expect the climbers to come in and warm up prior to the session, requiring the climbers to have set up junior Foundry Mountaineering Club Membership. There are three distinct levels of membership.

  • Blue Card: This allows the climber to come in prior to a TEAM Foundry coaching session and complete an agreed warmup routine using specific training equipment. This does not include climbing of any kind.

  • Green Card: This allows the climber to sign themselves into Toprope, Boulder and use Autobelays unsupervised whenever they wish. A staff member must assess the climber prior to this card being granted, once the climber / parent form has been completed. These are usually only awarded to ages 14 and over.

  • Red Card: This allows the climber to sign themselves into Lead, Toprope, Boulder and use Autobelays unsupervised whenever they wish. A staff member must assess the climber prior to this card being granted, once the climber / parent form has been completed. These are usually only awarded to ages 14 and over.

The same form is completed for both the Green and Red Card with lead climbing in the selected competencies.

 

Helmet Waiver

A helmet waiver allows lead climbing without a helmet. Completing this waiver is not essential however if you do require a helmet, please let your coach know when you lead climb.
